Help me with my Uni Project!

Hello. Im studying Advertising at University and one of my projects is to re-brand Waitrose' retro sweets in England. Im just researching what people think of Waitrose and also about sweets in general and need as many people as possible to fill in this questionnaire for me about 'sweets' (see link below).
